Loose skin is one of the most common reasons people come to see us, and one of the most often mis-sold. A lot of what gets called sagging is fat, and a lot of what gets called fat is skin that has lost its spring. So the first thing we do at our clinic in Whickham, Newcastle, is work out which one you are dealing with, because that decides which treatment will help and which would waste your money. Why skin loosens
Skin stays firm because of collagen and elastin, and the body makes less of both every year from your mid twenties. Sun, smoking, pregnancy, weight going up and down, and plain genetics speed the loss up, which is why the face, neck and jawline show it first and the tummy, upper arms, thighs and knees follow. Rapid weight loss, including weight loss injections, is the other big cause we see now: the fat goes and the skin that stretched over it is left behind. Every treatment below works the same way at heart, delivering controlled heat into the deeper layers so the existing collagen contracts and the skin builds new collagen over the following weeks.
The skin tightening treatments we use
HIFU (ULTRAcel Q+)
Focused ultrasound delivered to set depths beneath the surface, including the support layer a surgeon tightens in a facelift. It is the strongest non-surgical lift we offer and our first choice for jowls, a softening jawline, a loose neck and a heavy brow. Usually one session, with the result building over eight to twelve weeks and lasting twelve to eighteen months. Exilis Ultra Face
Radiofrequency and ultrasound together, heating the skin evenly and more gently than HIFU. It suits finer, crepey laxity, the skin around the eyes and the neck, and anyone who finds HIFU too much. Exilis Ultra Body
The same technology with larger applicators for the tummy, upper arms, inner thighs, knees and back, tightening the skin and reducing a little of the fat beneath it in the same session. A course of four is £785 for one area, £1,285 for two and £1,785 for three. See Exilis Ultra Body.
HIFU for the body
Focused ultrasound on the stomach, arms and thighs for skin left loose after pregnancy or weight loss, in one or two sessions, priced by area at your assessment. See HIFU body treatment.
Polynucleotides for crepey skin
Tightening firms loose skin; it does not fix skin that has become thin and crepey, which can look crumpled after a lift. Polynucleotides rebuild the skin's own quality at £250 a session, usually two to four sessions, and are the answer for crepey under-eyes and necks, often alongside HIFU or Exilis.
EMSculpt Neo where the muscle has gone too
A loose stomach after pregnancy usually sits over muscle that has stretched or separated, and no amount of tightening fixes that alone. EMSculpt Neo rebuilds the muscle underneath while reducing fat, £2,000 for a course of four, so the tightened skin has something firm to sit on.
Which skin tightening treatment for which area
Jowls and jawline: HIFU, with chin or jawline filler if the jaw has lost its edge. Neck: HIFU for loose skin, Exilis or polynucleotides for crepiness, and a combination for most necks. Brow and eyes: HIFU eyes and brow lift, or Exilis for the finer skin around the eyes. Stomach: Exilis Ultra Body or HIFU body for the skin, EMSculpt Neo if the muscle has gone, fat freezing first if there is a pocket of fat. Upper arms and bingo wings: Exilis Ultra Body, with fat freezing if the arm is heavy as well as loose. Thighs and knees: Exilis Ultra Body. Skin or fat? The question that decides everything
Pinch the area. If the skin lifts away easily and takes a moment to settle back, that is laxity and tightening will help. If you are holding a firm pocket, that is fat, and tightening the skin over it will do very little until the fat is dealt with. Most people over forty have some of both, and the honest plan is usually fat freezing or Emerald Laser first, then Exilis or HIFU to firm the skin once the fat has gone. Working out the order is what the free assessment is for.
Who it suits, and who it does not
Non-surgical tightening works best on mild to moderate laxity, where the skin has softened but still has some bounce. That covers most people in their late thirties to mid sixties, and most post-baby tummies and post-weight-loss arms. It does not remove skin. If there is a lot of overhanging skin after major weight loss, a machine will not match surgery, and we will tell you that at your assessment rather than sell you a course that cannot deliver.
Results and downtime
There is no downtime with any of these. You might be a little pink after Exilis, or slightly tender along the jaw for a day or two after HIFU, and most people go straight back to work. Results are not instant, because collagen takes weeks to rebuild: some tightening shows on the day, the real change arrives over eight to twelve weeks, and it keeps improving for months after a course ends. HIFU results typically last twelve to eighteen months; Exilis results are maintained with a top-up session between six and twelve months after the course. Safety
HIFU and Exilis are non-invasive, CE-marked treatments with no needles and nothing injected. The main risks are temporary: redness, mild swelling, tenderness and occasionally a small bruise or numb patch that settles over days to weeks. RT Aesthetics is a medically led clinic. Common questions
Does non-surgical skin tightening actually work?
Yes, on the right skin. Mild to moderate laxity firms and lifts noticeably; heavy, hanging skin does not, and no honest clinic will tell you otherwise. The assessment is where we say which yours is.
Which is better, HIFU or radiofrequency?
HIFU lifts more in one session and suits the jawline, jowls and neck. Radiofrequency, which is what Exilis uses alongside ultrasound, is gentler and works over a course, and suits the body, crepey skin and anyone who wants fat reduced at the same time. Our Exilis vs HIFU guide goes through it.
How many sessions do I need?
HIFU is usually one, sometimes two a few months apart for heavier laxity. Exilis is a course of four to six, one to two weeks apart. Polynucleotides two to four.
