What Exilis Ultra Face does

Exilis Ultra is a BTL device that puts monopolar radiofrequency and ultrasound through one small handpiece. The energy warms the dermis and the layer beneath it to around 40 degrees and holds it there while the applicator moves over the area, with built-in cooling so the surface of the skin stays comfortable. At that temperature the existing collagen fibres contract, so the skin firms a little straight away, and the skin is prompted to make new collagen over the following weeks, which is where most of the result comes from. On the face that means a firmer jawline, a smoother neck, softer lines around the eyes and mouth, and skin that looks denser rather than crepey. It is cleared by the US FDA for the non-invasive treatment of wrinkles and is CE-marked, with no needles, no anaesthetic and no downtime.

Areas we treat

The full face, for overall firmness and fine lines. The jawline and jowls, where the skin has started to slip over the jaw. The neck, for loose, crepey skin and the early turkey neck that HIFU can be too much for. The eyes and brow, where the skin is thin and the area too delicate for most tightening treatments, so Exilis is often the only option we offer there. The lips and the lines around the mouth. The décolletage, for the crepey chest that shows in a V-neck. Each of these is priced as its own area, and most people treat one or two.

What a session is like

You lie down, the practitioner applies a thin layer of gel, and the applicator moves over the area in slow, repeated passes. It feels warm, then hot, then warm again as the cooling kicks in, and most people describe it as a hot stone massage. There is no numbing cream because none is needed. A single area takes about thirty minutes and the full face and neck up to ninety. You can put make-up on and go straight back to work, and the skin may be a little pink for an hour or two. Sessions are one to two weeks apart, so a course of four is finished inside six weeks.

Results and how long they last

There is usually a small, visible firming after the first session, which is the collagen contracting, and it settles over the next few days. The real result builds as the new collagen forms, so the skin keeps improving for eight to twelve weeks after the last session, which is when we take your review photographs. With a top-up session every six to twelve months the result holds; without one, the new collagen ages at the normal pace and most people see the skin drift back over eighteen months to two years. Who it suits

Exilis Ultra Face is for mild to moderate laxity: the jawline that has softened rather than dropped, the neck that has started to crepe, the lines that are there at rest but not deep. It suits people from their late twenties, when the first softening shows, through to their seventies, and works on every skin type and tone, which is not true of every energy device. It is the tightening treatment we recommend to anyone who finds HIFU too much, and to anyone treating the eye area, where it is often the only option. It also suits people who would rather build a result over a course than sit through one intense session. It is not the right treatment for heavy jowls or a neck with a lot of loose skin, where HIFU does more, or for skin that has gone past what any non-surgical treatment can lift, and we will tell you that at the assessment rather than take your money.

Exilis or HIFU?

Both tighten skin without surgery and we offer both, so the choice is about how much lift you need and how much you are prepared to feel. HIFU sends focused ultrasound deeper, to the layer a surgeon tightens in a facelift, in one session that is uncomfortable but over quickly, and it gives the bigger lift for jowls, a heavy jawline and a loose neck, from £895 for the eyes and brow to £1,495 for the face, lips, neck and décolletage. Exilis works higher in the skin, more gently and over a course, and wins for fine lines, crepey texture, the eye area and anyone who finds HIFU too much. Safety

Exilis Ultra is a non-invasive, CE-marked treatment with a long safety record, and the applicator monitors the temperature of the skin throughout so it cannot overheat. Side effects are minor and short-lived: pinkness and warmth for an hour or two, occasionally a little tenderness or mild swelling the next day.
