Does HIFU Work? Results, Who It Works For and Why It Fails for Some People

Does HIFU work? For mild to moderate skin laxity on the face and neck, yes: one session gives a measurable lift that builds over three months and holds for a year or more, and it is one of the most booked treatments at our clinic for that reason. It does not work for everyone, it is not painless, and a lot of the people who say it did nothing were treated with the wrong device, at the wrong age, or judged it too early. This guide sets out what HIFU does, what the results look like and when, who it works for and who it does not, and what our clients in Newcastle say about theirs.

What HIFU does under the skin

High-intensity focused ultrasound sends sound energy through the skin and focuses it to a point, the way a magnifying glass focuses light. At that point the tissue heats to around 65 degrees for a fraction of a second, which does two things. The heated tissue contracts straight away, which is the small tightening you see on the day. Then the body treats each point as a tiny injury to repair and lays down new collagen around it over the following two to three months, which is the lift. The 4.5mm cartridge reaches the SMAS, the sheet of tissue under the skin that a surgeon tightens during a facelift, and that is why HIFU lifts rather than only smooths. A full face is a few hundred of those points in a grid.

Does HIFU really work? What the results look like

On the day, a little tightening, then very little for a few weeks. From week six the jawline starts to look cleaner and the skin denser. The peak is at about three months, which is when we take your comparison photographs, and some areas keep improving to six. For most people the result holds for twelve to eighteen months, and a single maintenance session keeps it. What it looks like in the mirror: a jawline that reads as a line again, cheeks that sit a little higher, a brow that opens the eye, skin under the chin that has pulled back, and a neck that has lost some of its crepe. What it does not look like: a facelift. The lift from one session is measured in millimetres, and millimetres are what most faces over forty need.

The reason so many of the reviews below were written at nine weeks is that nine weeks is when people start to see it. Judge HIFU at two weeks and you will decide it did not work, because it has not started yet.

Who HIFU works for

People over thirty-five with skin that has started to soften rather than drop: a jawline that has blurred, early jowls, a neck starting to crepe, a brow that sits lower than it did. Skin that is still firm to the pinch responds best, because HIFU rebuilds collagen and needs some to work with. Under thirty the skin is usually firm enough that there is little to lift, and the money is better spent on skin quality.

Who HIFU does not work for

Heavy jowls that hang, a neck in folds and skin you can gather in your fingers are past what any non-surgical treatment lifts, and a surgeon is the honest answer. Very thin skin over bone, some medical conditions and metal implants in the area rule it out or change the plan. Someone who wants volume back in hollow cheeks needs dermal fillers, not HIFU, and someone whose lines only show when they frown needs anti-wrinkle injections. We say which at the free assessment, and we turn people away when HIFU is not the answer.

Why HIFU did not work for some people

Four reasons cover nearly every "HIFU did nothing for me" story. The device: a clinical system such as the ULTRAcel Q+ treats at three depths with the energy set by a practitioner; the machines behind the £99 HIFU deals treat at one, often not deep enough to reach the layer that lifts. The candidate: HIFU on heavy laxity or on a twenty-five-year-old was never going to show. The timing: judged at two weeks rather than three months. The dose: too few shots over too small an area to save money, which saves the clinic time and gives you nothing. A clinic that gets those four right gets results in most people, most of the time, and that is what the reviews show.

Is HIFU worth it?

Against the alternatives, HIFU is the one treatment that lifts the SMAS without surgery, in one session, with no time off. Eyes and brow lift £895, double chin £995, full face £1,095, face and neck £1,295 at RT Aesthetics, with 0% finance over 12 months on plans of £1,000 or more, subject to a credit assessment. A facelift lifts further and lasts longer, for several thousand pounds, a general anaesthetic and weeks of recovery. Questions people ask

How long does HIFU take to work?

A little on the day, the real change from week six, the peak at three months. Some areas keep improving to six.

Does HIFU work on jowls?

On early to moderate jowls, yes, and it is the treatment we recommend most for them because it reaches the layer that has slipped. On jowls that hang, no. Our guide to whether HIFU can fix jowls goes through both.

Does HIFU work on the neck?

For crepey skin and the early turkey neck, yes, usually treated with the face so the jawline and neck lift together. For a neck in folds, a surgeon.

How many HIFU sessions do you need?

One for most people. Heavier laxity sometimes needs a second at eight to twelve weeks, and we say so at the assessment rather than after you have paid for the first.

Does HIFU hurt?

It is uncomfortable rather than painful: a hot prickle or a deep ache for a second per pulse, sharper over the jawbone and brow. The energy is turned down over the sensitive spots and there is no time off afterwards.

Does HIFU work for men?

Yes. Men come in for the jawline and under the chin more than anything else, and thicker skin often takes a little longer to show the result.
