Exilis Ultra: How It Works, Results, Reviews and Cost

Exilis Ultra is the treatment we reach for most often when someone's problem is skin that has lost its firmness, because it does something the other machines in the clinic do not: it tightens skin and shrinks the fat layer under it in the same session. This page explains how the Exilis Ultra 360 system actually works, what the science says, what a realistic result looks like, what clients at our clinic in Whickham, Newcastle, have said about it, and what it costs.

What Exilis Ultra is

Exilis Ultra 360 is a device made by BTL, the company behind EMSculpt Neo. It delivers two kinds of energy through one handpiece at the same time: monopolar radiofrequency and ultrasound. The radiofrequency does the heating. The ultrasound helps that heat spread evenly and reach deeper than radiofrequency manages on its own. The handpiece has cooling built in, so the surface of the skin stays comfortable while the tissue underneath is held at around 40 to 42 degrees, which is the temperature at which collagen fibres contract and fat cells start to break down. There are two applicators, a small one for the face and a large one for the body, and the practitioner moves it in slow circles over the area for the length of the session.

How Exilis works on skin

Heat does two things to the dermis. In the moment, it makes the existing collagen fibres shorten, which is why the skin looks a little tighter straight after a session. Over the following weeks, the controlled injury prompts fibroblasts to lay down new collagen and elastin, and that is the result that lasts. Because the energy is delivered in a controlled sweep rather than in single points, the tightening is even across the area rather than patchy, and the skin surface is never broken, so there is no downtime. Studies of monopolar radiofrequency on the face and body report measurable increases in dermal thickness and improvements in laxity scores over a course, with the effect building for weeks after the last session.

How Exilis works on fat

The same heat, held slightly longer and a little deeper with the body applicator, damages fat cells so they release their contents and are cleared by the lymphatic system over the following weeks. It is a gentler, slower fat reduction than fat freezing and it suits a different kind of problem: a soft layer with loose skin over it, rather than a firm pocket you can grab. That is why Exilis is the treatment for a crepey stomach after children or weight loss, for inner thighs and arms where the skin has gone slack, and for the look of cellulite, where tightening the skin and shrinking the fat under it both help.

Does Exilis work?

Yes, within its limits. Exilis produces a real improvement in skin firmness and a modest reduction in fat over a course, and the people who do best have mild to moderate laxity, a stable weight and the patience to finish the course. It is not a surgical lift and it will not remove a large volume of fat. Someone with heavy sagging will get more from HIFU on the face or from surgery, and someone with a defined pocket of fat will get more from fat freezing. What a course looks like

A course is four to six sessions, one to two weeks apart. A session takes 30 to 45 minutes for the face and about the same per body area. It feels like a hot stone massage: warm, sometimes briefly hot, never painful. The skin is pink for an hour afterwards and that is the extent of the downtime. Some firmness shows after the first session, the real change arrives from about four weeks in, and the result keeps improving for a couple of months after the course ends. It is maintained with a top up course when you feel it fading, typically after a year.

Exilis Ultra prices in Newcastle

Exilis Ultra Face courses of four sessions start at £475 for the lips, £775 for the eyes and brow, £975 for the neck, £1,075 for the face and £1,275 for the face and neck. Exilis Ultra Body is £785 for four sessions on one area or £1,199 for six, with two and three area courses priced on the Exilis Ultra Body page. Questions people ask

Is Exilis safe?

It is a CE marked medical device with a long safety record. The built-in cooling and temperature monitoring keep the skin surface protected. It is not used during pregnancy, over metal implants or a pacemaker, or over an active skin infection.

Exilis or fat freezing?

Fat freezing for a pocket you can pinch with normal skin over it. Exilis for a soft layer with loose or crepey skin over it, or for cellulite. Many clients have both, freezing first and Exilis afterwards to tighten.

How long do Exilis results last?

Around a year for most people, longer if your weight stays steady and you look after your skin, with a top up course keeping it going.

What is the difference between Exilis Ultra 360 and Exilis Elite?

Elite is the older radiofrequency-only version. Ultra 360 added ultrasound to the same handpiece so the heat goes deeper and spreads more evenly, which is the system we use.
