Most body treatments take fat away and stop there. EMSculpt Neo is the one we use when someone wants the area firmer as well as smaller: a stomach that is soft rather than fat, a bottom that has lost its shape, arms or thighs that need tone more than they need inches off. How EMSculpt Neo works

The applicator sits on the area and delivers two energies at once. Radiofrequency warms the muscle and the fat layer by several degrees in the first few minutes, the way a warm-up does before exercise, and takes the fat to a temperature at which fat cells break down and are cleared by the body over the following weeks. At the same time, high-intensity focused electromagnetic energy, HIFEM, contracts the muscle thousands of times in the session at an intensity you cannot reach by exercising, because it bypasses the limit your brain puts on a voluntary contraction. The muscle responds the way it does to hard training, by growing more and thicker fibres. One session is the equivalent of around 22,000 crunches or squats, without you moving.

Does EMSculpt Neo work? The numbers

BTL, who make the device, put EMSculpt Neo through seven clinical studies, and the averages across them were a 30% reduction in subcutaneous fat and a 25% increase in muscle volume in the treated area, measured three months after the course. Those are averages on people who fit the treatment: fairly stable weight, a soft or slightly fat area rather than a large one, and a full course of four. It is not a weight loss treatment and it will not shift a big pocket of fat on its own; for that we use fat freezing or Emerald Laser first. What a session is like

There is nothing to prepare. You lie down, the applicators are strapped over the area, and your practitioner sets the intensity to suit you and turns it up as you get used to it. You feel warmth building, then strong, rhythmic muscle contractions, which people describe as intense but tolerable, a bit like the hardest set of an exercise you have ever done without the effort. Each session is thirty minutes, and a course is four sessions five to ten days apart. Afterwards you go straight back to your day. Expect the same mild muscle soreness you would get after a serious workout for a day or two, and nothing else.

Results and how long they last

Muscle tone starts to show within a couple of weeks of the last session and the full result lands at about three months, which is when the fat clearance has finished too. The muscle you build behaves like muscle from training: it lasts for as long as you stay reasonably active, and a maintenance session every few months keeps it where it is. The fat cells that have been cleared do not come back as long as your weight stays roughly stable.

Who it suits, and who it does not

EMSculpt Neo suits people who are already close to where they want to be and want the last bit of tone that training has not given them, people rebuilding a stomach after pregnancy, and anyone who wants a firmer bottom without implants or filler. It suits slimmer people who want to build muscle as much as it suits people with a soft area. It does not suit anyone with a lot of fat over the muscle, because the contractions cannot reach through it and the result disappoints; we would treat the fat first. It is not used during pregnancy, over metal or electronic implants such as a pacemaker or copper coil, or over a recent surgical scar, and we go through your medical history at the assessment.

EMSculpt Neo or the alternatives?

Fat freezing removes more fat from a defined pocket but does nothing for muscle. Exilis Ultra tightens skin and reduces a little fat but does not build muscle. Surgery removes fat and skin but cannot build muscle either. EMSculpt Neo is the only one of the four that adds muscle, which is why it is the treatment for tone rather than size, and why it combines so well with the others.
